Working in partnership with the Rotherham Primary Care Trust (PCT)the United Multicultural Centre (UMCC) and the Eastwood & Springwell Gardens Neighbourhood Management Pathfinder (NMP), the Yorkshire Trust currently employs one of the two Community Development Workers for the Eastwood and Springwell Gardens Diabetes Project.
These employees have the remit to work along side the specialist diabetes Nurse to support individuals, their families, community groups and networks by providing information and support to encourage engagement with diabetes services and related support organisations.
The programme will enable Yorkshire Trust to work with households that include people with diabetes and their families to raise awareness and promote health within families and the wider communities. It will also enable us to work with stakeholder organisations to reduce the impact of diabetes and associated problems by promoting self care/management within an identified community supporting individuals in taking control of their health issue and making practical changes, acquiring good habits that will become embedded in the family and community.
